Some clinical trials for knee osteoarthritis have used sham surgeries—real incisions, real anesthesia, but the surgeon does nothing inside the knee. The results? Just as good as the real surgery. And those results hold up. It’s one of the most unsettling findings in modern medicine. Last week, we were looking for the universe’s missing matter. This week, we’re looking for a missing mechanism—the ghost in our own biology. A ghost is right. How can the expectation of a cure be as powerful as the scalpel itself?

That breaks my brain. It breaks the brain in the right direction, though. It forces us to stop thinking of the placebo effect as "fake." It's a genuine psychobiological event. When you believe a treatment will work, your brain doesn't just hope for the best. It gets to work. What does that mean, "gets to work"? Like, it starts telling itself a nice story? No, it starts releasing chemicals. Your prefrontal cortex and anterior cingulate—the parts of your brain that manage expectation and emotion—they light up.

And they trigger the release of the body's own painkillers, endorphins, and reward chemicals like dopamine. So your brain is essentially manufacturing its own drugs. Exactly. It’s running a predictive model. It anticipates relief, so it starts creating the conditions for relief. We can even prove it. If you give someone a placebo for pain, and it works, and then you give them naloxone—a drug that blocks opioid receptors—the placebo effect vanishes. Hold on. You’re saying the placebo was working on the exact same chemical pathways as an actual opioid?

That’s the point. It’s not a trick. It’s a different activation signal for the same system. The mind is literally changing the body's chemistry based on a story it's been told. And medicine has no idea what to do with this. Why not? If it works, it works. Harness it! Because you can’t bottle it. You can’t put "therapeutic context" or "patient expectation" into a pill. The entire model of modern pharmacology is based on finding a molecule that hits a specific receptor. As Harvard researcher Ted Kaptchuk puts it, doctors and scientists love molecules.

"They love that stuff." I get it. It's clean. It's cause and effect. This is... messy. It's profoundly messy. There is no single "placebo pathway." The mechanism changes depending on whether you're treating pain, or depression, or nausea. It’s a system-wide phenomenon. Where have we seen this before? A powerful tool that works, but we can't fully explain how? It reminds me of the early days of neural networks in AI. We could build a system, feed it data, and it would produce shockingly accurate results.

But for a long time, we couldn't peer inside the "black box" to see the specific, linear path of its logic. It was a complex, emergent property of the system. That’s a great parallel. We trusted the output, even if the process was opaque. Right. And that’s where the analogy holds. But here’s where it breaks. With AI, the mystery is computational. With the placebo effect, the mystery is consciousness itself. It's the story we tell ourselves becoming physically real. A 2026 study even found that the brain activity from an initial placebo response can predict long-term recovery in depression.

The belief itself becomes a biomarker for healing. So the effect isn't a bug in the data of clinical trials. It's a fundamental feature of our own biology. It might be the fundamental feature. The placebo effect isn't something to be explained away or controlled for. It’s the baseline. It’s the operating system for the human body. And we’ve spent a century trying to find a cure for our symptoms, when the cure might be baked into the system that perceives them. Which leaves us with the real question.

Not "how do we get rid of the placebo effect," but… how do we become skilled at using it? And what does it say about us that our own biology responds so powerfully to belief, to ritual, to nothing more than a story well told?
