In 2012, a study found that if you take biopsies from two different parts of the same tumor, two-thirds of the genetic mutations will be completely different. That's right. The cancer in your liver isn't one disease—it's a chaotic ecosystem of dozens of warring diseases. Last week on Science Showdown, we pitted brilliant minds against each other on the fundamental nature of physics. Today the question is just as fundamental, but far more personal: what is cancer? The textbook answer is simple: it’s a disease of bad luck and bad habits. It’s the Somatic Mutation Theory. The idea that environmental factors, or just random errors in cell division, create mutations in your DNA. Enough of the wrong mutations accumulate, and a cell goes rogue.
And for good reason! Ninety to ninety-five percent of all cancers are driven by these somatic mutations—changes acquired during your lifetime, not inherited. It's why we have precision medicine that can target a specific mutation and halt a tumor in its tracks. It works. It works... sometimes. But that simple story starts to fall apart the closer you look. For one thing, some of the most famous carcinogens in the world, like asbestos and arsenic, don't actually cause genetic mutations. Okay, but consider this—those are exceptions. The model is still the most powerful explanation we have. As oncologist Kristin Daly says, "The vast majority of biomarkers that we’re going to be testing for are going to be somatic." That’s where the action is.
I hear you, but what if the action is a symptom, not the cause? If the theory is right, cancer should be a linear process. Mutation A leads to B leads to C. But that’s not what we see. We see what you opened with—total genetic chaos. Well, the chaos is the result! One of the early mutations breaks the cell's own DNA-repair machinery. Once the editor is gone, the typos pile up everywhere. The initial mutation is still the first domino to fall. That's a neat story. But what if the dominoes were already unstable? What if the real problem isn't the individual mutations, but a catastrophic failure of the entire chromosome structure? A condition called aneuploidy. So you're siding with Peter Duesberg's argument—that the mutations are just passengers along for the ride?
That the real cancer-causing event is the large-scale scrambling of entire chromosomes? I'm saying his critique is powerful. He calls the somatic mutation theory "defective," and when you look at the evidence, you have to admit he has a point. The theory can't explain non-mutagenic carcinogens. It can't explain the spontaneous regression of some tumors. And it can't explain the sheer, unadulterated chaos inside a single tumor. Alright, I'll give you that one. The idea that we might be spending billions of dollars chasing the typos instead of the broken printing press... that's unsettling. It suggests our entire precision medicine paradigm is focused on the wrong thing. I don't think it's the wrong thing.
It's just... an incomplete thing. Look, no one is saying to stop testing for somatic mutations. For a patient sitting in a clinic today, that's the best tool we have. It saves lives. So we agree on that. For now, a combination of somatic and germline—the inherited kind—genomic profiling is absolutely essential. It empowers patients and doctors with the best information available. Exactly. We use the tools that work. But as scientists, we have to be asking if there's a deeper layer we're missing. We have to be open to the idea that the consensus might be wrong. The mutations are real, they are dangerous, and we can target them. But we still don't know for sure if we're treating the fire, or just the smoke.
